Family care teams are a new way to provide primary health care in Newfoundland and Labrador.

 

The vision for family care teams is to make it easier for people and families to get the health care they need in their own communities. Family care teams bring together different health-care professionals to work as a team to support peopleā€™s health and well-being.

Access to primary health care

Family care teams provide you with access to a variety of health professionals, including:

  • Family doctors.
  • Nurse practitioners.
  • Nurses.
  • Clinical and social navigators.

 

We also offer access to additional health providers based on your community’s needs and available resources, such as:

  • Physiotherapists.
  • Occupational therapists.
  • Speech therapists.
  • Pharmacists.
  • Social workers.
  • Public health nurses.
  • psychologists.
  • Community health nurses.

How to get connected to a family care team

If you are without a primary care provider or will be without one in the next 3 (three) months, you can be added to the provincial waitlist, called Patient Connect NL, to be connected to a family care team when one becomes available in your area.

Community partnerships

Our family care teams connect with local community-based practices, health centres, Indigenous governments and other health networks. We also collaborate with schools, municipalities, academic institutions and other community groups to provide you with complete care.

Health-care system improvement

Family care teams aim to improve health-care services in your community, making them more responsive to your needs. Our goal is to enhance your access to primary health care while addressing factors, like social issues, that can affect your health.

Each family care team is unique and designed to meet the specific needs of your community. Our family care teams are flexible, inclusive and able to adapt to changes over time.

You will be connected to a main provider on your family care team, who will make sure you receive the necessary care from the full team of health-care professionals.
