Care Coordination Centre (C3)

You are being transferred so you receive the care that best meets your medical needs. This may mean going to a hospital or site with specialized services, equipment or health-care providers.

The C3 team will work with the referring physician to arrange transfer to the closest hospital that can safely treat your condition and will also work with provincial paramedicine to coordinate your transport.  

Yes. No matter where you are in the province, you will receive the same standard of care.

All our health-care professionals are trained and qualified to provide safe, consistent care and held to the same rigorous standards.

Yes. Your care team at the receiving site can view your chart to see important details, such as your medical history, test results, and treatment plans.

You can request your health-care team to update your next-of-kin about your condition and transfer before you leave.

Once you arrive at the new facility, the receiving team will continue to provide updates to your next-of-kin.

If your condition has improved and you no longer require a higher level of care – but still need to return to a hospital closer to home – that medical transfer will be arranged at no cost to you. This is called repatriation and helps you continue your recovery closer to home.

Once you’re discharged home, you will need to arrange your own transportation.

There is no charge for transport if you’ve been admitted to an NL Health Services facility and require specialized in-patient medical services not available at that site.

However, when you’re ready to go home, you will need to arrange and pay for your own travel.
