Care Coordination Centre (C3)

Sometimes patients need to be moved to another hospital to receive the care they need.

The Care Coordination Centre (C3) coordinates in-patient transfers between hospitals and emergency departments across Newfoundland and Labrador. It works with doctors and specialists so patients can receive the care they need as quickly and safely as possible.

C3 is a centralized, RN-led service that operates 24 hours a day, 7 days a week.

Our goal is simple: Right care. Right place. Right time.

What this means for you 

  • Access the care you need, no matter where you live. 
  • Treatment at the hospital best equipped for your condition, as close to home as possible 
  • Support to return to a facility closer to home (where possible) when your medical condition allows.
